    Thank you for enrolling with Calvary Online School.  Please choose from one of the tuition payment plans below.  Be aware that completing this transaction will enact charges to your card.  If you are choosing a monthly payment plan, the plan will automatically reoccur on the same date each month as the original transaction. To change the date, or postpone payments, please contact the school after signing up for a monthly payment subscription.

Enrollment Agreement

Welcome to the family of distance learners at Calvary Online School. This Enrollment Agreement is intended to make the School’s relationship with You clear, and to set forth some of the School’s policies that will govern that relationship. If You have any questions or concerns that are not answered or addressed in this Enrollment Agreement, please call the Customer Service Department for further information. Calvary Online School partners with you to provide educational services and support for each student. This agreement ("Agreement") is between Calvary Online School ("School") and the Parent/Guardian/Student(s) Party ("You", "Customer") of any Calvary Online School’s products and services ("Service"). This Agreement governs all Services. By activating the Service, You acknowledge that You have read and understood, and You agree, to the terms of this Agreement. Calvary Online School may change the terms of the Agreement from time to time.

Enrollment Agreement
Tuition fees are based on either per-course pricing, one semester (6 courses) or full year (12 Courses). Additional courses may be taken but must be purchased in addition to the payment plan. Tuition includes curriculum, teacher and administrative services, college/career counseling options, education support service, ongoing customer service and maintenance of student records, evaluations and transcripts, etc.
Tuition or other fees may change due to enrollment specials or increase in fees.

Payment Plan
Calvary Online School families may pay in full at enrollment or take advantage of the School’s payment program of monthly payments over 10 months for annual enrollment, or five months for a one-semester enrollment.
• This fee is valued at a fixed amount per course. Paid in full or paid by monthly payments divided into 10 or five monthly payments (two semesters=10 payments, one semester=5 payments). Payment plan fees not paid in full are subject to a $10 processing fee per month for each month the fees remain unpaid.
• All payments are made from a credit card account or a direct debit from a bank account. A $25 fee shall apply for any insufficient funds.
• Since the School has reserved a space for the student, payment is due each month whether or not work has been submitted to the teacher.
• Enrollment is based on semesters. Students finishing a course or grade level in less time than planned are still required to pay the full tuition for the entire term. Final grades and/or certificates of completion will not be submitted until all outstanding charges are paid in full.
• Additional courses beyond standard enrollment (6 courses per semester or 12 courses per year) may be taken but must be purchased in addition to the payment plan.
• Calvary Online School reserves the right to suspend services if monthly payments are not made in a timely manner. A grace period of no more that 10 days shall be allowed. A re-activation fee of $50 will apply to all suspensions of service. If suspended, all tuition will be due to date and liable each month until a notice of withdrawal is received.
• Calvary Online School reserves the right to withhold issuing transcripts and/or diplomas for any student not in good financial standing with the School.

Extension Fee (only if necessary) Due in full at time of extension
A student who has not completed his or her course of study within the applicable period following enrollment (semester or year) may request an extension of the completion date, if eligible. A fee shall apply to each extension granted at a rate of $50 per course per week past the course completion date. Final grades shall only be given for work completed within the applicable term, as may be extended. In some extreme cases a full re-enrollment may be necessary.

Student Responsibility
Calvary Online School students are provided with ample opportunities, resources and instructional staff to succeed in this program. However, students still bear the weight of personally committing to their own academic success. Calvary Online School students are responsible for the following:
• Daily engaging in assigned academic studies (Daily engagement equates to attendance which is both mandatory and actively monitored).
• Completing all assigned tasks.
• Independently maintaining a successful studying environment.
• Asking for help when needed.
• Regularly communicating with assigned teacher(s) including a minimum of two forms of communication per week.
• Daily checking email correspondence and replying to all forms of communication in no less than 24 hours.
• Keeping parents/guardians informed of progress, grades, and communication from the assigned teacher(s).
• Following School rules and policies.
• Upholding a strong moral ethic with regard to academic honesty; especially in regard to cheating or plagiarism.

Parent Responsibility
To ensure a successful and rewarding distance learning program, each parent is an essential part of his or her child’s education. Nationwide studies show that students who receive regular support from their parents do better in school. Parents are responsible for the following:
• Contact Your student’s teacher(s) on a regular basis.
• Provide student(s) with access to an internet connected computer and a telephone
• Provide a suitable place for the student to study and help develop a workable schedule to complete assignments.
• Have funds available for monthly payments.
• Notify the School of any address, phone number and email changes or leave of absence.
• Notify the School if an extension or re-enrollment is needed to finish a course or grade level.
• Notify the School with a “Written Notification of Withdrawal” to withdraw from Calvary Online School or drop a course.
• Review and discuss online grade postings regularly with student(s).

What is Included
All enrollment packages include costs for curriculum, teacher and administrative services, college/career counseling options, education support service, ongoing customer service and maintenance of student records, evaluations and transcripts, etc.

What is Not Included
A one-time, non-refundable, enrollment fee of $50 will be added to all initial first-time enrollments to cover costs associated with enrollment. In addition, some classes may require additional materials such as art supplies or novels (if not available from a local library or online source). Courses requiring additional supplies will be noted and should be reviewed prior to enrollment to ensure adequate access to learning materials.
Withdrawal Refund Policy
Calvary Online School hopes You will be completely satisfied with the services and materials of Calvary Online School. If for any reason You are not satisfied, the School will refund Your money based on the terms and conditions described below.

75% of the tuition cost is refundable and will be applied to any remaining balance due, less the refund payment processing fees.

(For example, when the Customer makes an online purchase or enrolls in a marketing promotion, the School needs to retain certain personally-identifying information on the Site in order to respond to their request). A user's personally-identifying information may be used by the School to help us improve the website, and also for the School’s marketing and promotional purposes. The School receives and stores any information You enter on this Site or give us in any other way. You can choose not to provide certain information, but then You might not be able to take advantage of many features. The School uses the information that You provide for such purposes as responding to Your requests, customizing features for You, and communicating with You. Except as specifically stated herein, the School’s policy is not to share this information with third parties. The School will never sell this information to third-party marketing companies.

Information Sharing and Disclosure
Information collected through this Site may be supplied to affiliates of Calvary Online School, and other companies and organizations who perform work for the School under contract or sell products or services that complement the school’s products and services. The School otherwise exercises commercially reasonable care to not otherwise share or disclose the names of users or any personally identifiable information with third parties, except with the prior approval of the Customer.

Transferability of Credits and/or Diploma
Calvary Online School commits very seriously to working for a wide range recognition of its course credits or diplomas at outside agencies. However, like all schools, Calvary Online School is unable to guarantee universal acceptance of its credits or diploma at any outside agency, whether private or public. It is Your responsibility, as the parent/guardian, to ensure acceptance of credits or a diploma from Calvary Online School as desired.

You are fully aware that the materials provided to You are copyrighted materials. You agree to observe copyright rules and regulations such as not to copy and disseminate (or cause to disseminate) to any third party, any parts of the contents of the course material, in electronic format, in print, or otherwise. This paragraph shall survive termination of this agreement.

Considering the real time nature of the forums and chats, it is impossible for Calvary Online School to review all messages or confirm the validity of information posted.

Calvary Online School does not actively monitor the content of all posted messages and is not responsible for any messages posted. Calvary Online School does have some filters in place to notify and freeze posts or content that appears inappropriate, but the filters are not exhaustive and are not able to catch all inappropriate content.
Videos and images are to be submitted to teachers for approval before they are released onto Calvary Online School sites. Posts express the views of the author and are not necessarily the views of Calvary Online School.
Members remain solely responsible for the content of Your messages and behavior and You agree to indemnify and hold harmless Calvary Online School and its agents with respect to any claim based upon Your messages and behavior. Please be cautious in posting information of a personal nature or anything that might create legal concerns for You or others.

Calvary Online School does not provide monitoring of the site by any mental health professional and therefore is unable to determine if any student posting suggest physical or mental abuse, the use of illegal substances, illegal activity, the possibility of suicide or if a student poses a threat to others. Calvary Online School monitors are not mental health professionals and therefore may mistakenly identify and report such matters to Child Protective Services, law enforcement, parents or other authorities. By agreeing to use this site You accept the risk of such a mistake and its possible consequences and release and hold Calvary Online School and its agents, employees, etc. harmless of any liability arising from such a mistaken identification.

Customer agrees to defend, indemnify, and hold the School, its officers, directors, employees, owner(s), affiliated businesses, affiliates and agents and any other supplier and service provider who furnishes services to Customer in connection with this Agreement or the Service, harmless from and against all losses, expenses, costs, claims, including attorney’s fees or damages by, or in the right of, Customer relating to this Agreement, the Services, or the Device, or asserted by any third party due to or arising out of Your use of or conduct on the Service. This paragraph shall survive termination of this agreement.

Mandatory Arbitration
Any dispute or claim between Customer and Calvary Online School arising out of or relating to the Service or Device provided in connection with this Agreement shall be resolved by arbitration before a single arbitrator administered by the American Arbitration Association in San Diego County, California. Judgment on the award rendered by the arbitrators must be entered in the state and federal courts of San Diego County, California. All claims shall be arbitrated individually and Customer will not bring, or join a punitive or certified class action to arbitration or seek to consolidate or bring previously consolidated claims in arbitration. The arbitrator shall have no authority to award punitive damages. Customer acknowledges that this arbitration provision constitutes a waiver of any right to a jury trial.

Limitation of Liability
The School shall not be liable for any failure to provide the Service, at any time or from time to time, or any degradation of text, image or transmission quality that is caused by any of the following:
• act or omission of an underlying carrier or third party;
• equipment, software, network or facility failure;
• equipment, software, network or facility upgrade or modification;
• force majeure events such as (but not limited to) acts of god; strikes; fire; war; riot; government actions;
• equipment, software, network or facility shortage;
• equipment, software or facility relocation;
• service, equipment, software, network or facility failure caused by the loss of power to Customer; or
• any other cause that is beyond the School's control, including without limitation the failure of an incoming or outgoing call to be connected or completed, or degradation of voice quality and fax transmission quality.

The School's liability for any failure or mistake shall in no event exceed the fees student has paid the School (minus the enrollment fee of $50). The School is not liable for incidental or consequential damages of any type. All services provided by the School are provided “as is.” The School does not warrant that the service will be uninterrupted or error-free, or service will be corrected.

Calvary Online School will only maintain daily attendance records for full-time students. Students taking single courses or attending only part-time will not be counted in the school’s attendance records. The responsibility for part-time student attendance compliance with local/state laws will lie solely with the parent/guardian(s) of the part-time student and not with Calvary Online School.
In addition, the great variance in attendance laws from state to state and out of state necessitate that Calvary Online School will not be responsible for ensuring compliance with local attendance laws for full-time and/or part-time students. By enrolling in Calvary Online School, the student is enrolling in a California private school. The responsibility of compliance with local attendance laws lies solely with the parent/guardian(s).

If any provision of this Agreement is held to be invalid, void, or illegal by a court or arbitrator of competent jurisdiction, the same shall be deemed severable from the remainder of this Agreement and shall in no way affect, impair or invalidate the remaining provisions herein. If any provision shall be deemed invalid due to its scope or breadth, such provision shall be deemed valid to the extent of the scope or breadth permitted by law.
Participation in Calvary Online School is optional. Should You allow Your student to participate in Calvary Online School, You and the Student must abide by this Agreement.
